There is a lot you can do, but first step is a complete check up at the vets. It sounds as if the cat may have arthritis, in which case the vet can prescribe pain relief, and older cats often have other issues which are mostly very treatable. If you are in the UK and your grandmother has limited funds for vet treatment she may be eligible for help from the PDSA.

I can only repeat what fjm has said, there is a lot that can be done.

First a vets visit to try and find out why she can't jump, and whether walking is giving her problems. While you are there, if arthritis is diagnosed ask about supplements that might help, there are several available online that might help. I hate to ask but is she overweight and that is the reason she can't jump.

Second can you make any adaptions to your Grandmothers house that might help the cat get about. Can she jump at all, or even climb stairs. If she can then can you arrange things so she has a set of steps she can get up to reach windowsills, chairs, beds etc, places she liked to go but can't reach any more.

Finally, while 12 or 13 can be considered old for some cats with modern veterinary treatment cats can live well into their late teens (my own golden oldie is about 18, and when I took my Mother's cat in when my Mum had to move into a nursing home, she was 17 years old, arthritic and mostly blind, but still happy to wander about the house for a few months more (I think she missed my Mum and that is why she left us in the end rather than her age). Hopefully you can find out what is wrong and your Grandmother can enjoy her cat's company for a few years more.
