Anyone had a cat with gastrointestinal issues that took ages to diagnose properly?

Anyone had a cat with gastrointestinal issues that took ages to diagnose properly?

Our boys Moose is a 2 year old Russian Blue. For the past year, he's been intermittently vomiting/ has an upset stomach. He used to be big and healthy but has lost such weight.
We took him to the vet and they ran a blood panel - he was seriously deficient in vitamin B so we give him cobalaplex, which has helped.
He had an ultrasound which showed 'some inflammation' but nothing concrete. He had biopsies done which confirmed inflammation.
He was then put on 3 different kinds of steroids to help with this, none of which seemed to make a massive difference. We would take him to have a B12 booster shot at the vet every few weeks which was the only time he seemed to perk up. only for a few days through, then he would revert to a more subdued boy.
He's on gabapanthen twice a day but still cries almost once a day - you can hear his stomach working and churning as he cries, it's beyond heartbreaking.
We've had a food allergy test done which eliminated a few things which we've subsequently removed from his diet, and he no longer vomits or has diarrhea.
Yet he's still thin and apart from his moments of being his old, extremely affectionate self, he seems like a sad boy.
The vet says the next step would be exploratory surgery to manually check his GI tract, but he's so weak I really worry that height not survive the surgery.. Has anyone ever experienced anything like this (especially with such a young cat?)
