7 year old with bad gut

Hi, we have a 7 year old black long hair cat. She came from the shelter a few years ago so we only have so much history. She developed a bad gut a couple of years ago. Our vet said he could feel the constipation at one visit.
We took her off dry food and she's been eating only wet food. We also mix in some pumpkin, water and Miralax twice a day. After doing this she showed quite a bit of improvement. Mostly. The problem is about once a month she has an episode. It lasts for about an hour and she'll start by throwing up a few times. Then she'll poo. Then she'll poo more. By time it's over she will have thrown up multiple times and gone poo 8-12 times. By time she finishes the vomit is liquid and the poo is a clear gel-like substance (it's black when it starts). Oddly, while she always uses her litter box, during these episodes she goes to our showers to poo.
The vet has no idea (has suggested mega colon) and we don't really have time to get her in during an episode. Any chance anyone here has been through something like this and can offer an experience? We're over a year into this and are at a bit of a dead end. Thanks.
