Fat kitten - health issue

I have a kitten in breed The Russian Blue that is 6 months old. She is extremely fat and weights 4kilograms already. Her fat is hanging on her tummy. I am worried she might be sick, as just after a bit of walking or playing she gets tired and short of breath. Could it be her heart or she has problem with her digestion? Thank you. Marina
