Blood/Worm ? In cat stool

Hi, guys. Apologies for the grim first post but i am slightly concerned about my 11 year old Male, Gulliver.
He went and did his business as usual on his litter tray this morning but after i noticed this was outside the tray which looks like it may have dropped out after.
I am concerned as it appears to have blood in it and may even look a bit worm like?!
Does anyone know what this may be and if i need to be concerned and contact the Vet?
